Before You Begin
This symptom tracker is available in both Google Sheets and Microsoft Excel formats.
Google Sheets Version
Before entering any personal information, please create your own copy of the tracker:
Open the Google Sheet.
Click File → Make a Copy.
Give your copy a name and choose where to save it in your Google Drive.
Click Make a Copy.
Enter your information only in your personal copy.
This ensures your information remains private and prevents changes from being made to the original template.
Excel Version
If you prefer to use Microsoft Excel, simply download the Excel version and save it to your computer before entering any information.
